Lightning Talk Description: This presentation will provide knowledge and describe outcomes that contribute toward better care, value and education in community settings including focus on healthcare disparities. A review of a team-based approach to improve delivery of trauma informed, equitable, and evidenced based care for trans identified individuals in the rural MD-DE Salisbury Metropolitan area. The Transgender Interprofessional Care Team includes professors from social work, nursing, and psychology. A needs assessment to determine priorities was conducted including a community scan researching local and national resources to gain an understanding of existing resource infrastructure and community stakeholders. Those with lived experience have been integral to the development, operation, and outcomes of this project.
The two project outcomes included: 1) Exploratory needs assessment using focus groups and Zoom interviews with individuals from our transgender and gender non-conforming community. Goal was to collect their “stories” while also assessing gaps, barriers and needs to interprofessional health care. Aim 2 includes an Interprofessional Education Symposium with goals of highlighting the diverse needs of transgender individuals interacting. This symposium will bring together those with lived experience, professionals in rural community practices, and students in our health and human service programs. Pre-professional and continuing education inclue the development of clinical skills and competencies in providing gender affirming care.
Due to the health inequities related to gender affirming care from historical marginalization an integrated approach is necessary (Carson et al., 2021). Transgender experience and gender affirming care are often excluded from education and training. An emphasis has been often placed on sexual identities instead of gender differences rendering transgender care invisible in the healthcare system (Gahagan & Colpitts 2017). This project will promote increased visibility and education.
This presentation addresses the Quintuple Aims of enhancing patient experience, improving population health, increasing value, and improving provider wellness, and enhanced health equity. Attention will be paid to the needs and gaps to improve the impact on provider experience and patient centered care that is efficient, effective, and equitable. Promotion of institutional curricular and competency development of the IPEC core competencies will be included such as interprofessional values and ethics of promoting health equity across the lifespan, embracing the diversity of patients and the healthcare team, and addressing the impact on health outcomes. A call to action due to the onslaught of new legislation aiming to remove decision making power from both patients and providers.
